Peralihan fasa kerap merujuk kepada peralihan antara bentuk pepejal, cecair dan gas sesebuah jirim; malah juga plasma. Medium yang mengalami suatu peralihan ini pula berubah-ubah sifatnya secara terhenti-henti hasil perubahan kepada suaut keadaan luaran seperti suhu, tekanan dan sebagainya. Sebagai contoh - suatu cecair itu akan berubah menjadi gas apabila dipanaskan ke takat didihnya lalu merubah isipadu jirim sasaran secara mendadak, maka peralihan ini boleh dirujuk kepada penyukatan keadaan luaran ini sewaktu perubahan antara fasa jirim ini berlaku.
